Some local after school programs offer to pick up children at PS 58 at dismissal.
Parents:
Parents must submit the permission slip posted below before the first day of their child’s participation. This form must be given to the classroom teacher. It will be kept on file with our pupil accounting secretary in the main office. Parents must give the teacher advance notice in writing of any changes to a dismissal procedure. DO NOT SUBMIT THIS FORM FOR CARROLL KIDS PROGRAMS. This form is only for pick up BY OFFSITE programs at the end of the regular school day.
Offsite Program Staff:
Before you offer pick up service from PS 58 at dismissal, you MUST speak with the principal or parent coordinator before the first day to confirm that we have the capacity to support your group at the end of the day. Programs must:
- Provide the school a complete list of student names and days of participation.
- Provide the school with the name/s and contact phone numbers of your pick up staff.
- It is the program’s responsibility to distribute and make sure that PS 58 has a signed offsite dismissal permission slip for each child. Without a permission slip, we will not release the child to your representative.
- Your representative should arrive no later than 2:25 pm. They must show photo ID to our safety agent. Students will be waiting in room 101. Some larger groups may be waiting in the auditorium.
