Students in grades 1 to 5 must have a signed COVID testing permission form on file to be eligible for in-school testing

COVID testing of unvaccinated students and staff will continue this year.  Routine testing of 10% of eligible individuals will be conducted every week in all NYC public schools in the 2021-2022 school year.  Students in grades 1 to 5 whose parents have submitted consent for Covid testing for the 2021-2022 school year will be selected randomly.

If you gave consent last year, you must give consent again for 2021-2022 ASAP to avoid a lapse in eligibility.

NYC does not test students in grades PreK or Kindergarten at this time.

The easiest way to grant permission for COVID testing at school is through your NYC Schools account (NYCSA), at  Open the “Manage Account” menu and click “Consent Form”.
