All students (or their parents on their behalf), staff and visitors are required to answer the DOE Health Screening questions daily and have their temperature taken for access to our building. To speed up arrival, please add the HS tool to your phone.
Here are instructions for adding the mandatory DOE Health Screener to your phone. Please submit the form daily for each child before leaving home. This will help speed up arrival.

Here’s how to use it:
Each morning, open the HS shortcut on your phone’s homescreen and sign in using GUEST SCREENING.
Click “student” and enter your child’s name, and your email address.
To select our school, enter 058 in the search bar. Click on “P.S. 058 The Carroll School” in the drop down menu.
Answer the health questions.
Show us your phone’s screen displaying the “good to go green” arrow. If you have multiple children at PS 58, capture an image of the final screen for each child and show them at arrival.
